I Have Small Cuts On My Penis On The Top

I have small cuts on my penis on the top around my foreskin. This keep happening every so often. I clean this area well and I am uncircumcised. I have used coconut oil to moisturise the area and it starts to heal but it's been over a month now and hasn't really healed properly. Can you suggest a cream that will help?
