I Have Sporadic Pain In My Breast Behind The Nipple,

I have sporadic pain in my breast behind the nipple, deep within breast. I just recently had a diagnostic mammogram that found a 7cm mass, and the ultrasound rated it a birad 4. It recommended further follow up. How concerned should I be that this could be malignant?
