I Have Terrible Chest Pains. It Goes All The Way

I have terrible chest pains. It goes all the way into my back, stomach, neck, teeth, and chest. The only way I can find relief is ice cold water. It's been happening for months. Sometimes it happens days in a row for hours at a time. I went to the ER because it was so bad and they couldn't find anything after X-rays and blood work. What could this be?
