I Have This Pain In My Upper Arm That Comes

I have this pain in my upper arm that comes and goes but is now
Ia very sharp pain. I can move it but it has to be in a certain position NOT to hurt. I took two ibuprofen an hour ago and it seems worse.

I want to know should I ICE it or put heat on it.
