I Have Type 2 Diabetes Which My GP Told Me

I have type 2 diabetes which my GP told me is poorly controlled. A recent HBA1C result was 59.
I have asthma, fibro, sleep apnoea and Hallux rigidus as well, which I googled and found that they can cause high blood sugar levels. I'm on cymbalta 120 mgs, Crestor 10, singulair 500, desunin 10 and co-diavan, which can all cause high blood sugar.
Any suggestions as to high I can ever get my hba1c back to the mid 40s, where it was up to this time last year?
