I Have What Feels Like A Scab On The Top

I have what feels like a scab on the top of my outer ear. It's been there for like a year. I pick it with nail clippers thinking it will go away. But it doesn't.
And when I sleep on it , it hurts.
Is it time to see ear nose and throat doctor.
And what is your opinion
