I Need To Make A Dr. Appt. And Don't Know

I need to make a Dr. appt. and don't know what to ask doctor to check. I have many various problems, many which may be age-related, but only am prescribed Ativan for anxiety. Haven't had check-up for many years. Blood pressure was borderline. Smoker who needs to quit but trying always results in serious anxiety & depression. Too fatigued and low to do daily routines, or eat healthy. What tests should I ask doctor for?
