I need to understand the protocol for how to ensure continuation of coverage after patient abandonment i.e termination of 8 yrs of MD patient-doctor care with a transfer to any other MD -lead to an absence of what is now required - formulary wise - to allow me fulfillment of medications I have been taking at the same mg and same amount daily since 2009. Because of the lack of a new MD when my prior doctor terminated our work, I was able to only acquire care from a med manager who refuses - after 4 years of prescribing the same medication - to fulfill the formulary & prerequisites in order to allow the pharmacy to approve script and to allow my new insurance to cover the cost.
