I Received An Email From MDVIP Saying That I Should

I received an email from MDVIP saying that I should request an appointment with Dr. Deckert. I made the contact and they told me that I have not filled out a questionnaire at the time of my wellness exam (which was indeed completed). Where do I find the form I should complete?
