I Want To Ween Off Topirimate After At Least 12

I want to ween off Topirimate after at least 12 years of taking 100 mg daily. It doesn't seem to help anymore and cannot tolerate 200mg. The side effects, at 70, have gotten worse. How can I safely ween off this medication? Thank you for your help. I've googled, but to no avail. Trudy
