I Was Released From The Hospital 5 Days Ago After

I was released from the hospital 5 days ago after a 6 day stay for pneumonia. Was still tired and achy but doing well until 2 days ago when I got a terrible case of heartburn. I did have heartburn in the hospital but a couple chewable tabs took care of it then. Now I'm taking Tums waiting for OTC nexium to work but it is extremely painful to even swallow water. I have a slight fever, 99.4 and my chest, collar bones all the way down to my fingers ache badly. What should I do?
