If Your Doctor Changes The Dosage Of Your Pain Medication

If your doctor changes the dosage of your pain medication prescribed by my old primary care provider (where I was stable) to a much lower dosage and has changed it twice and yet it has not properly relieved the pain. How do I continue stating my cause without him thinking I am just abusing these narcotics? I just want to be where I was previously.
