Ihave Had Back Problems Numerous Times Over Last 40 Yrs.I

Ihave had back problems numerous times over last 40 yrs.I have gone to chiropractor which usually solves the problem for awhile I'm now in so much pain I feel like I'm in labor but pain is continuous.In my lower back radiating to my hip an down my leg Nothing is relieving the pain despite hot an cold compresses an pain meds.Is it time for me toconsider surgery?
