Im 25 Years Of Age And For The Past 3

Im 25 years of age and for the past 3 months my foreskin has become a little tight. I can pull it back but I get some rips on the foreskin. Plus some times when I finish having sex my foreskin swells a little bit. What could have cause this to happen. And what can I do for my foreskin?
