I'm A 50 Year Old Female. Native American. Was Diagnosed

I'm a 50 year old female. Native American. Was diagnosed June of 2014 with type 2 diabetes after losing 40 pounds. Another symtoms I had was my hair falling out in huge clumps. Started taking 5 metformin a day plus Lantus at night. About 3 months ago I stopped taking both mess as they were causing me stomach problems. I've been now feeling very tired all the time and it's been painful around my lower back and sometimes front. Very uncomfortable. I can also feel my heart hurting occasionally to the point I feel like I'm having a heart attack.
