I'm A 59 Year Female Who's Post Menopausal For 8

I'm a 59 year female who's post menopausal for 8 years. I get recurrent vaginal and urinary infections.
I currently take vagefem twice a week. My OBGYN prescribes fluconazole, along with steroidal and anti fungal creams. This gives me temporary relief, but then my vaginal itching and burning return.
Suggestions?
