I'm Amost 63, Female And Was Diagnosed With Type 2

I'm amost 63, female and was diagnosed with Type 2 Diabetes in Oct 2014. I wasn't significantly overweight, but have since lost 11kgs and My BSL/BGL readings have been consistently in the hight 4's and low 5's for the past 12 months. My question is, how often is it advisable to visit my medical practitioner? Every 12 months, for a blood test, or only when I have a medically-related problem? TIA
