Im Having Sharp Pains In My Right Side/hip...aching In My

Im having sharp pains in my right side/hip...aching in my abdomen. .and i have been spotting/ bleeding every 2-3 days now..meaning ill go from period to stopped for a few days then spot for a week..then stop for a few days..repeat...ive had a mirena for a little over 4 years...yesterday and today i woke up in severe abdominal pain...stabbing feelings..attempted using restroom only causing severe stabbing pain...this has lasted about 2 hours after waking eventually wanning into a dull lower back ache and right hip pain that last all day....what does this mean...?
