In A Recent Blood Test My Tsh Level Was 4.93,

In a recent blood test my tsh level was 4.93, I'm 54 years old and I'm on meds for high blood pressure which is averaging 130/84 for past two years. I am extremely tired all the time even with additional sleep (although I have not been able to sleep well for a couple of months), as well as weakness throughout my body. I am suffering with stress headaches daily, this was diagnosed 2 years ago and and take Effexor which worked great until 2 or 3 months ago. Should I be concerned with this tsh number?
