Information Was Provided By A Physician Or Other Appropriate Health

Information was provided by a physician or other appropriate health care provider familiar with my clinical situation and decided to put the arteriovenous fistula in my right arm. I agreed for a fistula to be place in my right arm; however when I woke up in the recovery room placement of a loop graft was place in my left arm. The graft never did worked since its placement two years ago. What do I do?
