Is Discharge Of Sperms At Night And While Urinating A Cause For Concern?

Hey Doctor Me Zainy 23 years old and I m masturbating since last 9 years but now for 2 years I have a controlled on myself but the major problem with me is that I cum out,all sperm or semen come out from my penis and while sleeping Nocturnal ejaculation,how to get a rid of it firstly and how to avoid ejaculating naturally without any solid reason out at night .Secondly I also discharge some sperm or semen while urinary time and it s really feel me down and disgusting for me.Although this problem started with me since last 1 and a half year I reckon.My penis size is almost 4 cm and full erected maximum 5.5 or more.So is it healthy size and good for intercourse.So kindly help me out DOCTOR.Any tips and whatever you can better advice me DOCTOR.I ll be so kind of you.
