Is Fluid In The Mastoid Air Cells Anything To Be

Is fluid in the mastoid air cells anything to be concerned about when there is no sign of inner ear infection, but there is chronic ear pain. Have had multiple ruptures and multiple tube placed in both ears. Have had a chronic headache X 2 weeks. Also have hx of SLE.
