Is Having Fatty Liver, Enlarged Spleen And 15mm Portal Vein During Pregnancy Safe?

Hi I am 26 years old and pregnant for 36 months I have a fatty liver from before pregnancy , my gastroenterologist ordered an ultrasound of liver and spleen during pregnancy during pregnancy spleen enlarged to 14.5 cm my doctor then ordered a doppler of portal vein to check for enlargement of portal vein results were portal vein 15 mm my Dr has ordered endoscopy possible necessity of caesarean section please give opinion if this is necessary and what else should be done
