Is Irritation Around The Stoma With Blood Clots Post Bladder Removal Normal?

I had my bladder removed in 2013, recently I vv_hope27@e had a lot of irritation around the stoma and a lot of blood clots. I know clots are normal, but I have a clot on the side of my stoma I tried cleaning around my stoma but it is not going anywhere. On further inspection it kind of looks like my skin has torn some around my stoma. Should I be concerned or just call my urologist in the morning?
