Is It Common Practice For Plastic Surgeons Who Do Accept

Is it common practice for plastic surgeons who do accept OHIP qualified patients for breast reduction, to charge an additional fee? Why isn't OHIP covering the full cost? Isn't it unethical to offer "two standards of care" dependant on an additional $5,500?
