Is It Normal To Have Tight Scrotum And Small Penis In A 9 Year Old?

My great nephew is 9 yrs old and his strotum stays drawn tight and his penis is very small. They took him to a specialist and they said that it was because he was overweight, but I know other men that was heavy and this did not happen to him. His parents are very passive and won t push the issue but I am very concerned. If they are something wrong I think mentally it would be easier to address it now than during pruberty. Do you have any suggestions?
