Is Persistent Scrotal Pain With Pyospermia After Varicocelectomy?

Hello, i had varicocecelomy operation in 2019 and i test sperm count after surgery the volume was normal but with pyosprmia. before and also now i fell pain by touching scorotum but ultasound was normal no finnnding for inflammation please advise me what should i do .is it dangerous
