I've Been Experiencing Pain, Sleeplessness, Short Term Memory Loss Progressing

I've been experiencing pain, sleeplessness, short term memory loss progressing over the last 6 months. Mainly torso pain to start, but now pain in my left forearm and frequent headaches. I have never had headache issues my whole life. Fatigue. I'm having a hard time sleeping with the pain is probably a factor here for sure. Dreaming a lot more than I used to. Vivid images from dreams even when I am just trying to take a nap and not really asleep, or so I think. That is hard to describe. I've been taking 800mgs of ibuprofen 3-4 times a day for near 5 months. It helps for an hour or so after about an hour or so of taking it. I'm 40. 6 foot 290 lbs male. I've gained about 50 lbs over the last 6 months from not being as active. I smoke. I have since I was 19. I drink beer,wine, vodka on occasion, probably more than I should, and a lot more recently to help with sleep. I have no insurance. I've had a couple family members suggest I go to the ER.
