I've Been Having Blood In My Stool And Bleeding Like

I've been having blood in my stool and bleeding like I have a heavy period out my rectom. Even if I pee before I start to push out the rest of my urine I'm bleeding a lot of bright red blood.itsthe same liquid like a cheap red wine flowing down without a bowl movement. Just last February I had a hysterectomy because after 40 some years of heavy periods and rectal bleeding one had to stop I couldn't handle 2-3 weeks of bleeding and when 1 stop the other end would start . But it still bleeding alot more clots a starting to fall out now. I'm scared because it's red like the color of a rose . Can help me with some advice on what to do?
