I've Been Having Pain In My Right Lower Back For

I've been having pain in my right lower back for about 8 hours. I've taken aleve, and ibuprofen but nothing is helping. I don't have my appendix and my period was 2 weeks ago. The pain has caused me to black out twice. WhT could it be and when should I go to the er?
