I've Been On 200mcg Fentanyl Patch For 17yr. Dr Cut

I've been on 200mcg fentanyl patch for 17yr. Dr cut to 175 every72hrs. For 2 months. Then cut I t to 100mcg/72hr. 12to15 hrs. Before patch is changed I get severe restlessness, muscle/joint pain,insomnia. I' called dr.but only get nurse.she says can't talk to Dr. Can't get more fentanyl(i don't want more fentanyl!). I'm 70yrs. old, have crohns, copd and high blood pressure. Do I just have to gut it out?
