I've Had PsA For 38 Years. Had Both Feet /

I've had PsA for 38 years. Had both feet / all toes reconstructed along with hand/ finger surgeries. My feet were about 10 years ago & now are back to very eroded & very painful. The pain in my feet is nearly unbearable & now it's also painful at the very tips of both big toes. I receive Remicaide 600mg every 6 weeks & some pain medication for occasional use. I'm wondering if the pain at the very tips of my toes is something I should be concerned about - bone death? Is that a thing that would cause so much pain right there?
