I've Had Some Long Standing Shoulder Pain That Radiates Down

I've had some long standing shoulder pain that radiates down my arm from the tip of my shoulder down the front of the arm as well as laterally. I've tried creams like Ben Gay and heat which if they help it's only for a few hours. It's painful enough that I cannot sleep on that side.
