I've Spent Years With Shoulder Pain That Started Around Age

I've spent years with shoulder pain that started around age 12. I am now 27. The last several days, I've been going hours where my fingers will be numb. Generally from my middle finger down to my pinky. Raising my hand above my heart seems to bring feeling back to it but I'm worried as often as this has been happening the last few days that it going untreated for years may have caused further damage. I've never visited a doctor for this for several reasons, usually comes down to having the money for it or just having a busy life. I guess I'm looking to find out how necessary nerve treatment would be for something like this. Any help is appreciated, thank you
