Just Received My MRI Results For Rt. Shoulder. Have Had

Just received my MRI results for Rt. shoulder. Have had continued moderate to severe pain x's many months. Have tired one cortisone injection, relieve for about 3 weeks, then several PT sessions with little change.
MRI impression shows: Low grade partial tearing anterior supraspinatus tendon at the footprint in a background of severe tendinosis
Mild subacromial/subdeltoid bursitis
Small shoulder joint effusion

What is recommended??
