Male Age 65. Heart In Afib. Put On Xarelto For

Male age 65. Heart in afib. Put on Xarelto for blood thinning. MRI on brain shows 2 mini strokes. Also told he has advanced microvasular changes in brain. On blood pressure medicine and cholesterol medicine but labs looked fine. Not a diabetic. What is course of action?
