Many Years Ago I Took Maxolon And Was Highly Allergic

Many years ago I took Maxolon and was highly allergic with head spasms or movements (my head flicked right back and I was rushed to hospital. I am only now applying for a medic alert bracelet, what would the engraving be on that - maxolon or the ingredient in it?
