My 12 Year Old Son Has Trouble With His Nipples

My 12 year old son has trouble with his nipples turning red and being very irritated after swimming in a standard pool, salt water pool, and the ocean. What could be the cause and how do we treat it. He is overweight so his breasts are about an "A" cup or 1" extension from his body. He always wears a shirt in the water.
