My 1year Log Grand Son Has A Rash All Over

My 1year log grand son has a rash all over his body from head to feet. He's had it for a over a week and we are waiting for a referral by his PCP to see a dermatologist. Today he is crying in agony and I can't get him to stop because he is itching so bad. Should I take him to urgent care or ER.
