My 2 Year Old Son Has One Nipple That For

My 2 year old son has one nipple that for a few weeks now has been hard, he is touching it more and more and pulling at it like it bothers him. Tonight while he was in the bath I noticed that side of his chest is swelling. What are the possible diagnosis of these? As he is only 2 I didn’t suspect gynomastia
