My 95yo Mother In Law Is Losing Her "self Awareness"

My 95yo mother in law is losing her "self awareness" . Recently, she offered to loan me one of her dresses. She is 4'8, 103lbs. I am 5'8, 145 lbs. Yesterday she wanted to go to the store to get wrinkle cream for her face. She noticed she had wrinkles and was surprised. She has spinal stenosis, COPD, CHF. She fell in September and broke her right hip. She lives in an assisted living facility.
