My Grandmother (74 Y.o.) Is Having Some Health Problems And

My Grandmother (74 y.o.) is having some health problems and we are trying to help her get to the right Dr.
Within the past month, she has lost about 35lbs (she was losing 2lbs a day for at least 2 weeks). She went from about 155-160lbs to now 125lbs; she is 5'1. Her taste has been off; she states food does taste like she knows it should, if she eats anything with a type of seasoning, it is amplified. "food with salt taste like a salt block"; same for sugar. She is barely eating which isn't helping because everything taste bad and she puts up mental blocks instead of forcing some food down. Some times it seems like a mental block trying to get her to like the food she is eating.
Additionally, in the past week, she has had a loss of depth perception and weaking of the right side of her body. She is also very forgetful and slurring words.
Her last blood workup showed vitamin deficiencies in potasium, iron, magnesium, B12, and calcium (all of which she has been taking supplements of for 3 weeks now) and they are not improving.

About one year ago, she had some issues with sever hair loss on the top of her head and seemed to be getting slightly forgetful; like where did I put my car keys or glasses but nothing alarming.
