My Almost Three Month Old Daughter Has Had Diarrhea For

My almost three month old daughter has had diarrhea for the past three days it started out yellowish, creamy and thick like it has been (breast feeding) then started getting green, watery, seedy and some slimmy. I have been giving her one formula bottle a day to give my breat a break, I'm getting worried because it's not stopping normally she has one to two poopy diapers a day but it has been more then five a day
