My Blood Pressure Has Been Running From 135-140 Over 90-92.

My blood pressure has been running from 135-140 over 90-92. Heart diseases run in my family. My grandfather passed away from a heart attack and my father needed a heart transplant due from his heart attack. Should I be concerned with this blood pressure reading I'm getting due to heart disease that runs in my family? If so what would you recommend
