My Bmi Is Much Over 30, Actually Around 45 Or

My bmi is much over 30, actually around 45 or so, I had nexplanon inserted in 2015 and it's not "scheduled" to be replaced until 8/18. But since my bmi is higher than 30 should I have it replaced sooner? Is it less effective? Is there a better method besides oral contraceptives that I can use? I can only use progesterin only methods because of the other medicines I am on.
