My Brother Died Suddenly. A Witness Said He Began Foaming

My brother died suddenly. A witness said he began foaming at the mouth and collapsed. His defibrillator did not go off until after he hit the floor. CPR to no avail, he did not regain consciousness. Why didn't the defibrillator restart the heart? And what caused the foaming?
