My Cat Has Lost An Extreme Amount Of Weight And

My cat has lost an extreme amount of weight and has lost hair on many parts of her body. She appears to have puncture marks and open sores on her back quarters. She is an outside farm cat and perhaps another animal attacked her. Any ideas? Thank you.
