My Close Friend Has A Fish Like Smell To His

My close friend has a fish like smell to his breath. He is a very clean man, brushes and uses mouthwash regularly. Even after eating Altoids it's so hard to kiss this wonderful sweet man. I don't want to hurt his feelings but sometimes the odor is so strong it turns my stomach. What if anything can I do?
