My Cousin Is In A Rehabilitation Center After Been In

My cousin is in a rehabilitation center after been in the hospital. She needs to move to a long care facility very soon. She has Parkinson's for many years, her feet are twisting and also her legs starting to bend too. She is physical disable since 2013. She was at the Palace for some years but her income lowered and she could not afford it anymore. She is also mentally disable not able to make decisions. I am trying to help her.
How can I find a facility for her? She lives in Miami, Fla. her income is 2800 a month it might go lower if she loose her divorce benefits. She is 72 years old.
