My Dad Is 53 Yrs Old And Is Leaking Clear

My dad is 53 yrs old and is leaking clear and sometimes a poop color fluid and sometimes poop comes out even when he's not going to the bathroom it has irritated the skin to the point of burning the skin around this area kinda like diper rash any help on this problem is greatly appreciated.
